Your guide to a great handover

If you're leaving your role at UHB, whether you're moving to a new position, moving to a UHB+ (Bank) only position or leaving the Trust, creating a handover is an important part of the process. It helps your team stay on track and makes things easier for the person stepping into your role. 

Think of it as passing the baton, you're helping make sure nothing gets missed and your work continues smoothly. 

Why a handover matters 

  • It keeps your team informed and prepared
  • It helps your manager understand what’s outstanding
  • It supports a smooth transition for whoever takes on your responsibilities

What to include in your handover 

Here are some helpful questions to consider when creating a great handover

Tasks and responsibilities 

  • What are your main day-to-day tasks? 
  • Are there any ongoing projects or deadlines we need to be aware of? 
  • What tasks will need immediate attention after you leave? 

Meetings and events 

  • Are there any regular meetings you attend or lead? 
  • Are there upcoming events, reviews, or deadlines we should prepare for? 

Contacts and communication 

  • Who are your key contacts (internal and external)? 
  • Are there any relationships that need to be maintained or handed over? 

Team and management duties 

  • Do you have any line management responsibilities? 
  • Are there upcoming appraisals, absence reviews, or check-ins scheduled? 

Systems and documents 

  • Are there any documents or folders that need to be transferred? 
  • Is access to shared drives, Teams channels, or systems up to date? 

Tips for a smooth handover 

  • Keep it clear and simple (bullet points work well!)
  • Organise your files and folders so they’re easy to find
  • If possible, talk through your handover with your manager or team before your last day

Patient handover 

  • Identify all patients allocated to the member of staff leaving and re-allocate those patients to another member of staff 
  • Check if there is a need to redirect any test results to another member of staff to review/be aware of 
  • Check if you need to inform the patient about the changes in staff looking after their care
