360-degree feedback

The 360-degree feedback process can be used to gather wider feedback on an appraisee’s behaviour and should be undertaken around the time of their appraisal. It is to be used to help identify areas that require support in order to improve, and to give positive feedback where appropriate. 

Page two of the 360-degree feedback tool is to be completed by the appraisee.

Page three should be completed by colleagues, as per the instructions in the tool. These colleagues must send completed feedback to the appraiser, and not to the appraisee themselves.

The feedback should be collated by the appraiser, then discussed in the appraisal meeting, and the summary at page four completed.
